Warranty

Axon Systems' Neurodiagnostic Accessories products are warranted to be free from defects in workmanship and material under normal use for a period of 60 days from the date of delivery. If you are not satisfied with any product purchased from this catalog, we will replace it or credit your account if we receive the goods back (unopened) within 60 days of purchase. This warranty is extended only to the original purchaser.

Terms of Payment
Net 30 days with approved credit. A finance charge of 1.5% per month (18% annually) will be applied to overdue invoices. For your convenience MasterCard, Visa and American Express credit cards are accepted.

Shipping and Handling
In most cases your order will be shipped the same business day. Orders placed after 3:30PM will ship the following business day. Orders are shipped UPS ground. Three Day Select Service, Preferred Delivery Service may be requested at the time the order is placed. Shipping, handling and insurance charges are prepaid and added to your invoice. All shipments are F.O.B., Hauppauge, NY.

Back-Orders
In the unlikely event that any of your items are back-ordered, at your option, we will ship the available portion of your order. The remaining items will be shipped as soon as they are available. Shipping and handling charges will not be applied to the second shipment caused by a back-order situation.

Sales Tax
All prices exclude state and local taxes. Sales and local taxes will be applied where applicable. If you are tax exempt, please send us your tax exemption certificate.

Returns
If you are returning merchandise, please contact the Accessories Department by phone at 800.888.2966 or by Email to sales@axonsystems.com for an return authorization number (RMA). All returned items are subject to prior approval and must be accompanied by the RMA number. Returned items must be in their original condition and packaging, clean and undamaged to qualify for credit or replacement. Ship items with a copy of the original invoice. Returns received after 60 days of purchase will not be accepted. Returns may be subject to 15% restocking fee.

Liability
Axon Systems, Inc. is not liable for any damages resulting from the use or misuse or caused by a defect, failure or malfunction of the product. Federal law restricts the sale and use of these products to, or on the order of a licensed practitioner.

Sterilization (of reusable products) and application are the the responsibility of the user. Presterilized, disposable products are intended for Single Patient Use Only.

Axon Systems reserves the right to change product specifications and prices without notice. We are not responsible for typographical errors.
